Rose Tree Order Form - North East Surrey Crematorium

Throughout our Gardens of Remembrance you will find rose beds with roses available to dedicate for a period of 5 years. Each rose dedication comes with a plastic marker plaque included in the cost.

Alternatively you may wish to purchase a larger slate plaque to display at the foot of your rose dedication (there is an additional cost for a slate plaque).

A maximum of 2 dedication plaques are permitted for each rose lease.

I wish to purchase a Standard Rose with a plastic marker plaque for a dedication period of 5 years.

The plaque can accommodate 6 lines of inscription. The recommended maximum number of letters and spaces is 20 on each line, any greater number will result in thinner / smaller lettering which causes visible distortion in the finished plaque.

I wish to purchase a Standard Rose with a Slate Plaque for a dedication period of 5 years.

The actual size of the slate plaque is 6” x 4” with 8 lines of inscription without a motif or 6 lines of inscription with a motif. Motifs appear at the bottom of the inscription. The recommended maximum number of letters / spaces is 30 on the dedication lines, any greater number will result in thinner / smaller lettering which causes visible distortion in the finished plaque.

IMPORTANT- PLEASE READ THE CONDITIONS SET OUT BELOW:

The person paying is permitted to dedicate the rose and plaque for 5 years from the date of payment. The rose may be renewed at the end of the dedication period subject to the regulations then in force. A reminder letter will be sent to the address we hold on file. It therefore important that any change of address is notified to us. If the dedication is not renewed the plaques will be removed.
A maximum number of 2 name plaques can be placed at each rose dedication.
To enable us to keep the gardens in the best possible order, it is necessary for the staff to remove flowers, plants, vases or any other items placed on lawns, rockeries or in rose beds weekly. Where possible items will be stored for 3 months for you to collect.
The Board reserve the right to alter the variety of plants/plaques or the design of the gardens as they may deem necessary.
